Kohai denotes a person junior to you within an institution: a younger student, a newer colleague, a club member who joined later. It is the reciprocal of senpai and the two only make sense together.
Unlike senpai, kohai is rarely used as a form of address. You would refer to someone as your kohai when speaking about them, but address them by name. This asymmetry is easy to miss and is a common error in fan writing.
The junior is expected to show deference and to learn; the senior is expected to teach, protect and take some responsibility for them. It is a genuine mutual obligation rather than a one-way hierarchy.
Sports manga uses this structure heavily, since a club with successive year groups produces the relationship automatically — and produces the graduation that ends it, which is one of the genre's reliable emotional mechanisms.
A character shifting how they address someone is a plot event. Dropping a formal term, or beginning to use a name directly, marks a change in the relationship that the dialogue may never state outright.
